Referral Services

For a referral to another medical office or specialty office, please call our office 401-773-7901, and leave the following information:

  • Your Name
  • Date of Birth
  • Your phone number
  • The name of your insurance company
  • The name of the specialist*

Referrals take 2 to 4 business days to process. Please be aware that some out-of-network or out-of-state referrals can take up to 30 days to process.

* If you can also provide your specialists address, phone number, and tax number and when and why you will be seeing them

Wednesday 8:30 am – 6:00 pm
Thursday 8:00 am – 3:00 pm
Friday 8:00 am – 3:00 pm
Saturday: Morning Appointments Available Please Contact Office

New Patients - Before your first visit, please complete the necessary registration forms and arrive 15 minutes before your scheduled appointment time.

Cancellations

If you arrive to your scheduled appointment late, you may need to be rescheduled. This helps to ensure that each scheduled patient can be seen in a timely fashion. We request 24 hour’s notice on all cancellations. If you cancel your appointment with less then 24 hours notice or if you miss your appointment you will be charged a fee.
